Valley Health Partners at Community Action Development Allentown: 523 N 7th Street, Allentown.
A partnership between Valley Health Partners (VHP) Community Health Center and Community Action Development Allentown is providing community members social support services on site through the help of a Community Health Worker.
VHP Staff will be available from 1:00-4:00pm every Tuesday afternoon, no appointment necessary.
Community members may receive assistance with the following:
-COMPASS applications (an online tool for Pennsylvanians to apply for many health and human service programs and manage benefit information)
-Supplemental Nutrition Assistance Program (SNAP) benefits
-Connection to a Primary Care Physician (PCP)
-Housing and rental assistance resources
-and more!
